Riverside Casino and Golf Resort is partnering with Mercy Medical Center in Cedar Rapids to provide customized wellness services for you. In order to ensure these programs will be of benefit and value to you, we need your input.
Please take a few moments to complete this assessment. Upon completion of the online Health Risk Assessment (HRA), you will receive a written report from Mercy Fitness Center that includes your online HRA and clinical results.
The assessment will ask you questions about your personal well-being. Your responses are completely confidential, so please answer honestly. Riverside Casino and Golf Resort will not have access to any individual responses. Responses will only be reported to Riverside Casino and Golf Resort in a summary report with anonymous group data.
